In consideration of being allowed to participate on behalf of Glacier Sports, Inc./aka Ohio Glaciers (hereinafter “Glaciers” and/or “Releasees”) athletic program and related events, tournaments and activities, all players must fill out the form below

To register a player, please download the registration form using the blue button and follow these instructions carefully.

  1. Please read the player registration contract fully
  2. Please fill in the information on Pages 1, 2, 4, 6, 8
  3. Once filled out, please save the copy to your computer
  4. Once the file is saved, please email a copy to [email protected]
  5. Once completed, your child may start team activities at our facilities
